Changing addresses on Driver's licences, Health Cards and Outdoors Cards

To change your address for the Ministry of Transportation, the Ministry of Health and Long-Term Care and the Ministry of Natural Resources, you will have to either go to a Service Ontario centre in person or to use their website. It will take approximately four to six weeks for you to receive your new card.

Changing your name

To change your name, you'll need to go with your marriage certificate and your current photo health card and/or driver’s licence to your nearest Service Ontario centre.

Applying for an Ontario Photo Card

If you are over 16 and you can't drive, the Government of Ontario provides photo identification cards that can be used as legal identification for government services. To apply for an Ontario Photo Card, please visit your local Service Ontario Centre.

Organ and Tissue Donation

Did you know that by donating your organs after you die, you could save as many as 75 lives? If you are over 16, you can register to be an organ donor. The process can be done entirely online. All you need is your health card.

Accessible Parking Permits

Interested in an accesible parking permit? To apply for an accessible parking permit, you'll either have to go to a Service Ontario centre in person or mail your application and any supporting documents to Service Ontario.
